It can include vital signs (physical, functional, emotional, & social), medication adherence, symptoms, and lifestyle habits.
This document can serve as a basis for discussing the effectiveness of the current treatment plan, identifying areas for improvement, and setting new health goals. Additionally, it can facilitate open communication and collaboration between the patient and physician, ensuring that the treatment plan is personalized and effective.
